What is Community Health Assist Scheme (CHAS)?
The Community Health Assist Scheme (CHAS), formerly known as the Primary Care Partnership Scheme (PCPS) was created by Singapore Ministry of Health (MOH). In the past, patients were issued the Community Medical Benefits (CMB) cards. Now patients are issued the Community Health Assist Scheme (CHAS) cards. Those with Public Assistance (PA) Card are automatically included.

How and where to use CHAS Card : 
CHAS Health Assist Card covers selected dental treatment, acute conditions and chronic diseases covered under the Chronic Disease Management Programme (CDMP). The CHAS card can be used at some private clinic and dentist. These General Practitioners (GPs) provide common outpatient medical and Dentist provides dental treatment to patients at subsidised charges.

Age eligibility: 40 years and above

Income Criteria: $800 to $1,500 per capita monthly household income.

Application Form: Applicants can collect the CHAS Application Form at any Restructured Hospital, Polyclinic, Community Centre and Club (CC) or Community Development Council (CDC).
